  • Personal Statement explaining circumstances for the damage
  • Sworn Affidavit to support (iii) above
  • Police Abstract for loss of Passport
  • Copy of the lost passport bio-data page, if available
  • Payment of the prescribed fee of Ksh.12,050.00
